Entrustment of Personal Information Processing
Entrustment of personal information processing refers to the delegation of certain tasks to specialized contractors to provide smooth services to "Members." Personal information is processed only within the scope entrusted under the responsibility of the "Company." 전문 의료용 인공지능(AI) 기업 휴런(대표 신동훈)은 허혈성 뇌졸중(뇌경색) 의심 환자의 뇌손상도를 점수화한 Alberta Stroke Program Early CT Score(ASPECTS)를 비조영 CT 기반으로 자동 산출해주는 Heuron ASPTECTS가 최근 인용지수 8.572의 SCI급 국제학술지 Journal of NeuroInterventional Surgery에 게재되었다고 밝혔다.
ASPECTS는 뇌경색 환자의 뇌 손상 정도를 판단하는 방법으로, 10개 해부학적 영역 별로 뇌 대혈관폐색에 따른 손상 유무를 분류하고 뇌손상의 정도를 0-10점의 점수로 환산 제공하는 방법이다. ASPECTS는 현재 응급 뇌졸중 환자의 초기 분류에 활용되고 있는데, 점수가 낮을수록 뇌 손상의 범위가 넓다는 것을 의미한다. 하지만 ASPECTS는 임상적 효과성 이면에 판독자의 숙련도에 따른 판독자간 점수 오차가 크다는 문제점이 항상 제기되어 왔다. 따라서 휴런은 판독자간 판독 오차 최소화하며 임상 현장에서 의료진의 진료에 도움이 될 수 있도록 숙련된 전문의 판독 수준의 ASPECTS 자동 산출 인공지능 솔루션을 개발했다.
휴런의 연구진과 아주대학교의료원 신경과 이성준/이진수 교수와의 협업으로 이루어진 이번 연구는 허혈성 뇌졸중이 의심되어 응급실에 내원한 19 세 이상의 환자 326명 대상으로 Heuron ASPECTS와 전문가의 합의도를 분석하여 진단 정확도를 평가했다. 연구 결과, Heuron ASPECTS는 전문가의 판독 결과와 우수한 합의도 및 재현성를 보였다. 특히, 응급 대혈관폐색 환자들 중 혈관재개통 시술 대상 환자 선별에 활용될 수 있는 ASPECTS 점수 기반 이진 분류 결과 4점 초과인 환자들을 분류하는 성능은 뇌졸중 전문의 합의 판독 결과와 94% 이상 일치했으며, 6점 초과인 환자들을 분류하는 성능은 95% 이상 일치하여 높은 정확도가 확인됐다. 이러한 결과는 Heuron ASPECTS가 보다 객관적이고 정량적인 점수를 제공하여 의사 숙련도에 따른 결과의 차이를 최소화할 수 있고, 판독 소요 시간을 줄일 수 있어 임상에서 신속한 치료 결정에 유용한 솔루션으로 활용될 수 있음을 시사한다.
뇌졸중 ASPECTS 점수를 자동산출하는 프로그램은 이전에도 있었으나 응급실에서 허혈성 뇌졸중 식별과 유사한 상황에서 예측 능력을 검증한 연구는 Heuron ASPECTS가 처음이다.
해당 연구를 주도한 아주대병원 신경과 이진수 교수는 “이번 연구는 Heuron ASPECTS의 예측 능력이 10년 이상의 임상 경력의 전문의의 판독 결과와 일치도가 높다는 것을 입증함과 동시에 비조영 CT 단계에서 응급 환자선별에 도움을 줄 수 있다는 유의미한 결과를 확인할 수 있었다”며 “특히, 뇌졸중 전문의가 부재하거나 한정된 인력과 자원으로 환자 진료에 촌각을 다투는 응급실에서 빠른 의사 결정을 도와 환자의 예후를 높이는데 기여할 것”이라고 말했다.
